Reasonable Adjustment Digital Flag

Under the Equality Act (2010), organisations have a legal duty to make changes in their approach or provision to ensure that services are as accessible to people with disabilities as they are for everybody else. These changes are called reasonable adjustments.

The Reasonable Adjustment Flag was developed to enable health and care workers to record, share and view details of reasonable adjustments across the NHS, wherever the person is treated. They are documented and viewable to any staff accessing your medical records.

Reasonable adjustments may be required for all manner of reasons, for example sensory issues (e.g. using sign language or braille), mobility issues (e.g. needing to use a wheelchair, only being able to walk short distances), or learning disabilities where someone may have difficulty processing information or need a carer present with them.

At Chew Medical Practice we are in the process of identifying patients who may wish to have a reasonable adjustment added to their record. We may contact patients by text message or discuss this with patients during a routine appointment. Patients may also request a reasonable adjustment to be added to their record where appropriate.
